How to Register with the practice

NEW PATIENT QUESTIONNAIRE Please complete the New Patient Questionnaire and return the completed form to the surgery along with your GMS1 form.  You will be contacted and a New Patient Health check will be arranged for you.  YOUR REGISTRATION WILL NOT BE COMPLETE UNTIL YOU HAVE ATTENDED FOR YOUR NEW PATIENT HEALTH CHECK.

You have the right to opt out of having a summary care record. If you are certain you do NOT want a summary care record, please download the opt out form and return it to the surgery.

Mentell provides a safe space for you to talk and share openly with like minded individuals. It is completely free of charge and meetings last for two hours. All you have to do is turn up with the desire to make progress – whatever that means to you.

The health and social care services of Hyde are holding a ‘Healthy Memory Cafe’ at The Hub, Stockport Road, Hattersley, SK14 6NT for a chat and a cuppa on the last Tuesday of every month between 2pm and 4pm for advice and guidance about your memory. You will be able to talk to health and social care professionals, legal advisers, volunteer organisations, Alzheimer support workers and more. whether you have a dementia diagnosis or not, we are there of offer help and guidance about your memory.
